CHECK - IN

Every team is required to check-in prior to their first game.

Consent Form MUST be emailed to tim.memmer@valentisathletica.com.

Required Items:

• Medical Consent Forms for each player (Your school's form). You will need to have these on hand, we are not collecting these.

• 795 Cup Invitational Consent Form (Please - 1 form/team)
